New to the Practice and Ready to make an appointment?
Click on the Request Appointment button below. Select “Initial Psychiatric Diagnostic Evaluation”, then select the location you prefer. Next, select the available date and time that works best for you. Provide your contact information and click the “Request Appointment” button. I will review your request and may contact you to gather a little more information before we confirm the appointment.
Existing clients wanting to schedule an appointment?
Click on the Request Appointment button to request a link to your Client Portal. Once inside your portal, select the Request Now button under the Appointments tab. Select the Service, the Office location and the available date and time that works best for you. I will confirm the appointment or suggest an alternate date or time.
If you are a Returning Client whose last appointment was more than 6 months ago, please Contact Me. Your account will need to be reactivated before you can access your Client Portal.
