Whether you are considering an “In Office” or an Online “Telehealth” Video session, our Online Appointment Request service is a convenient way to schedule an appointment. Click on the Appointment Request button. Select the session type, the location, the date and time your prefer and enter your contact information. We will contact clients who are new to the practice to gather a little more information before we confirm the appointment.
After we’ve confirmed our appointment, you will receive a message that contains a link to the Client Portal. You will have access to several digital documents or forms that you can complete electronically before we meet. These upfront procedures allow us to focus more of out initial session time on you.
There is no need to print and complete any forms or sign documents. You will have access to policy and procedural documents through the Portal.
